You’re right that “know” is being overworked in my initial statement. But, insofar as DGBI is a broad church of mechanisms from stimuli to neuronal function to the brain, and DGBI is the terminology employed by the body defining IBS, there really aren’t any major competing theories any more.

I respectfully have to take issue with this statement.

We know that modulation of neurochemicals can modulate GI symptoms, and/or perception of GI symptoms in certain patients and we know that there is a connection between certain kinds of gut-brain signaling, but to say that this is the original causal mechanism behind IBS isn’t founded. It’s one proposed model of IBS, one of many.

Just proving the concept that these pathways are active in GI symptom perception doesn’t prove that this is the root cause. It merely proves that these pathways can be leveraged as potential treatments.

Might want to leave that choice to the people having to decide. My friend had her whole lower body opened in an abdominal exenteration surgy. She had multiple abdominal organs removed, vagina, as well as vertebrae. She had to relearn how to walk, and now has an ostomy bag and catheter. Some people turn down the surgery because they would rather die. My friend is happy she did it. Some pain is worth getting to spend extra years on this planet. That's what most terminal cancer patients want. More time to get to the next treatment, to hopefully find a cure or long term solution. Living with stage 4 cancer is not all pain and suffering. Most people are just living their lives.

Right? I am also extremely wary of any article that uses the term 'cancer' with no qualifier. Which cancer? Which subtype? Breast cancer alone has like 4 subtypes with different genetic mutations driving those as well. Pancreatic cancer is not lung cancer is not leukemia. 'Cancer' is broad a term its almost meaningless except to describe something as apposed to another totally different disease type.

When my condition was first looked at, they said it could be MS or a brain tumour too- those were scary times

When I was travelling and went to Indonesia, where weed is really illegal, I went 7 days without smoking and was by day 7 into constant spasm. A king pin on the island took pity, heard the issue an gave me a joint, amd told to to walk out into the ocean to smoke it away from everyone and not be caught. I was back to normal before I finished the joint. It's my godsend! I'm glad you found yours too!
